New details regarding Patrick Muldoon’s health prior to his untimely death have been revealed less than two weeks after his fatal heart attack.
According to his death certificate exclusively obtained by Us Weekly on Wednesday, April 29, Muldoon’s immediate cause of death was myocardial infarction, commonly known as a heart attack.
The underlying causes included hereditary coagulopathy disorder, a genetic condition that impairs the blood’s ability to form clots properly. Pulmonary embolism was also listed as an underlying cause, which is a blood clot that travels to and blocks a vital artery or the lungs.
Muldoon was ultimately cremated on Tuesday, April 28, according to the death certificate released by the County of Los Angeles’ Department of Public Health. The certificate also lists his occupation as actor and producer and outlines that he was never married.
The Days of Our Lives and Starship Troopers actor died on April 19 shortly before 11 a.m. following a heart attack at the age of 57. After his manager confirmed the news, many costars and close friends shared powerful tributes and emotional memories about Muldoon.
Muldoon’s close friend and ex-girlfriend Denise Richards was one of the many stars to share a heartfelt message online.
“This is so hard for me to put into words. I am deeply heartbroken & devastated losing you. You were my best friend & my family,” Richards, 55, wrote via Instagram on April 21. “We met in our first acting class, I was 19 never had an acting job and you were 21 a student at USC doing a recurring role on Who’s the Boss.”

The actress continued, “The friendship, love, support, respect & loyalty we had was unconditional and rare.”
Tori Spelling also paid tribute to her ex with a personal note on social media.
“I wasn’t ready to write because then it felt real. And, this shouldn’t be real,” she wrote via Instagram on April 21. “What I met and ultimately fell in love w/ was not just a beautiful on the outside creature but the most beautiful soul. He was special and one of a kind. The kindest most caring human w/ raddest weird sense of humor. We got each other! Laughed a lot ! I laughed w/him and at him and he loved it.”
Although Muldoon was never married, he was in a committed relationship with his girlfriend, Miriam Rothbart, at the time of his death.
“My girl. Two years 2gether today from the night this picture was taken where I got you drunk enough to become my girlfriend,” he wrote via Instagram in August 2025. “Thank u 🙏 my ❤️.”
Rothbart has yet to publicly comment on her partner’s death.
